DMV: Traffic Accident Report Form. Question?
Question by Umi008 : DMV: Traffic Accident Report Form. Question? I was wondering if the first section the traffic accident report, “reporting party’s information,” refers to me as the driver or whoever I was in the accident with? I crashed my car by myself, so there was no other parties. In the form there is 4 section: reporting party’s information, other party’s information, injury/death damage, and your vehicle insurance. Do I just fill out the section “your vehicle insurance” and leave the rest blank since there was no one else involve?

the officer that came to the scene should have filled out any and all info needed on the form
you should not write on it at all

Yes – you are the reporting party, and since there wasn’t another party you would put n/a in that spot, but check with your insurance company first to be sure they haven’t already filed one on your behalf. BTW – you or your insurance company notifies the DMV – not the police.
